Perfectionist seems to have issues @sprsoldier96 but your profile has it credited to you.

Some quests don't activate right away.

Umm, there seems to be an error with the 'Perfectionist' achievement... In the sense that I haven't completed half the achievements and have just received 'Perfectionist'... I shouldn't even have it yet?

I have experienced the same problem as above. I got the quest around the halfway point, so told Nichodemus who recommended posting here and linking @MrDayCee.
