Have you already used it? You might want to read up on the new update on how to use it from Con yesterday.
If it just disappeared maybe you could file a bug report with them. I'm not going to use mine until people post about it over there.
The item you are trying to upgrade must be of the same level specified and of the same quality. Maybe include a screenshot of your token and the item you tried upgrading?
