Can be anything, from bosses to gltches.

Mine was in resident evil 4 when the flaming zombie burst out of a refrigerator, or when the iron maiden stalked me.

Gears of war I think it was. Those crazy things that run at you... I was half asleep and out of no where it lets out this big screech and kills me.

Yes! No... Well, sorta. It wasn't "scary" no. In fact, it was a quite boring chase, but I remember it being terrifying the first time I played. As for Saw-X, she is terrifying.
