You need to beat the game once with any of the heroes. You will unlock new game plus and cave of trials. the cave of trials involves a huge army of enemies, with a titan unit. That titan unit actually shows up outside the cave of trials, depending on which character the last town you don't control is. There's a fire unit, which resembles 'The End' from Epic War 2 (except not so tall it goes off the top of the screen), an ice unit called 'blue dragon', a thunder titan, whose name eludes me, and a shadow titan. There is 1 in each of hte first 4 caves. The 5th cave contains an inviniclbe gemlin that instantly kills any unit it hits. You will require phoenix and ice to get past that one. And it's a good idea to use phoenix and ice to weaken the other titans so your units can kill it. In fact, for the third cave, I took a screenshot with the fire and ice titans in it.
The caves get a lot of money and exp whether you win or lose, so when I start off with a new character I always head there a few times to bulk up my mana by levelling. (winning a cave usually gets me VERY close to level 30, which is the max.) It is possible to have up to 3 of the titans out at once on your side, without dying from lack of units. (They take 200 seconds when fully upgraded for the fire, ice, and thunder titans to actualyl get out on teh field. Even longer for the shadow titan)
