Are the results of trial and error translated into fact or theory?

The test must be repeted several amount of times and if the results are always the same then it is a fact.It is atheory when it hasn't been proved.

Actually it is a hypothesis until it is proven, through multiple tests and trials which result in identical information. A theory is a collection of facts and laws which explain a particular subject, such as evolution, gravity, etc.
