This is actually a hard choice for me. Out of all games, I'd have to pick Kingdom Hearts. The game makes no sense unless your like an uber fan of the whole thing. I've played it, and even the first one looks confusing. The trailers make no sense at all. There are probably some games with worse storylines, but I don't know any others.
I would say Call of Duty Black Ops, partly due to the fact that I skipped the cut scenes, in a nutshell it's some guy was hypnotised by the Russians or whoever and made to kill J F Kennedy and half of it is him sitting in a chair having flash backs and somehow this Russian guy who he thinks is real is all in his head but somehow tells him how the Russians got hold of this gas and were attacked by the British when the British found out they gassed Nickoli I mean WTF. That probably isn't even it, it's just so random.
Black Ops storyline annoyed the hell out of me. But it wasn't my game so i returned it and didn't waste my money on it. Hopefully MW3 will be better, cause the cliffhanger in the second one was good. But really CoD is only bought for the multitplayer. XD
Black Ops Had a really confusing storyline,I don't even know the nationality of half the people.
My personal favorite for bad storylines would have to be Katamari Damacy. It's a really fun game, but the reason that you're running around with a magical glue ball is because your dad drove his car into the moon. And ALL of the stars exploded. That's right, ALL the stars. And for some reason, the task of putting the stars back together fell upon his 5 centimeter tall son.
The Black Ops storyline is one of the worst out there, but I think there are games with worse.
The game I've played with the worst storyline would have to be the newest Super Smash Bros. game. It doesn't make any sense, and it's just random people attacking you for now reason. It doesn't even explain how all of the characters got put into one univers or why they are fighting eachother. It's just a series for Nintendo to milk what money they can get from the handful of fans who still follow them for more than just the next Legend of Zelda game.
true... the mario games have a pretty... lazy storyline (notice how i didnt say storylineS). but the thing is... they arent trying to have a good storyline.

the fact that you cant understand a storyline doesnt make it bad. i actually loved it and i might even say its one of the best.
the worst for me would be final fantasy 12. because it has a horibble story although its TRYING to have a good one. nothing really happens there... and when something happens... well you forget whats the last thing that happened.
I gotta say that Dark Souls has the worst storyline I've seen so far. To be fair, I'm only 10 hours in but the shallow attempt to have a storyline in this game is just awful. You constantly meet random characters that say the most random things, no two which seem to ever be tied together in any way.
