Each character of a username has 37 different possibilities. The 20-character usernames alone have more than 1x10^48 possibilities, so that will never be an issue.
That probably won't ever happen. Kongregate has over 6 million accounts (probably 7 million now) and there are still common usernames that I could make on that site. I don't think you could run out of usernames if you could always use underscores and numbers, and even then, you could always look up words in other languages.
